Reporting a Birth, Marriage, Divorce, or Death

When there is a birth, marriage, divorce, or death, a report must be filed with your Ward Office. Please direct all inquiries to your local ward office.

1. Birth of a Child
You must file a report within fourteen days after the birth of a child.
Necessary Documents:
- Birth certificate issued by the hospital
- Mother and Child Health Handbook (Boshi kenko techo)
- Your personal seal (inkan), if you have one



2. Marriage
After you get married, you must file a report of marriage.
Necessary Documents:
- Certificate of marriage issued by the ward office
- Attached documents. (The filing process may differ depending on your nationality. Please consult your local ward office.)


3. Divorce
After you get divorced, you must file a report of divorce.
Necessary Documents:
- Certificate of divorce
- Attached documents. (The filing process may differ depending on your nationality. Please consult your local ward office.)


4. Death
Within seven days of a death, a report of death must be filed. The procedures for applying for a burial will be taken care of at the same time.

Necessary Documents:
- Death certificate issued by the hospital

- Your personal seal (inkan), if you have one.
